Case from 2014–2017 (U.S. Attorney's Office latest release: April 27, 2017). Published on Scammers.com Oct. 16, 2026; case status is current as of that date.

Charles S. DeHaan, a suspended physician from Belvidere, Illinois, was sentenced on April 27, 2017, to nine years in federal prison for defrauding Medicare, including by billing for visits to patients who had already died, the U.S. Attorney's Office for the Northern District of Illinois announced [1]. He was ordered to pay $2,787,054 in restitution.

How the scheme worked

According to DeHaan's written plea agreement, as described by prosecutors [1] [2]:

  1. House calls: From January 2009 to January 2014, DeHaan mainly billed Medicare for in-home patient visits and for certifying patients as homebound.
  2. Upcoding: He billed routine, non-complex visits at Medicare's highest reimbursement levels when he knew they didn't qualify.
  3. Visits that never happened: He billed for patients he never treated. Some of the patients had died before the date of the claimed visit.

The 2014 indictment said DeHaan was president of Housecall Physicians Group of Rockford, S.C., and treated patients at Rockford-area assisted living facilities, where it alleged he obtained Medicare beneficiaries' information without their knowledge or consent [3].

The person sentenced

Charles S. DeHaan, 62 at sentencing, of Belvidere, Ill. [1]

  • Arrest (Jan. 24, 2014): arrested on a federal complaint; his medical license had been suspended that month [4].
  • Indictment (Feb. 11, 2014): nine counts of health care fraud [3].
  • Plea (May 20, 2016): pleaded guilty to two counts of health care fraud before U.S. District Judge Frederick J. Kapala [2].
  • Sentence (April 27, 2017): nine years in prison, three years of supervised release and $2,787,054 in restitution, imposed by Judge Kapala.
  • Investigation: FBI and HHS Office of Inspector General, with help from the Illinois Department of Financial and Professional Regulation.

Some early Justice Department releases spell his name "DeHann."

Red flags

  • Medicare statements listing home visits that you or a relative didn't receive.
  • Visits billed after a patient's death, which families can spot on a deceased relative's final Medicare summary notices.
  • A provider asking assisted-living staff for patient lists of people he hasn't treated.

What to do if you're targeted

  1. Review Medicare Summary Notices or your account at medicare.gov for services you don't recognize.
  2. Check a deceased relative's final notices for claims dated after death.
  3. Report suspected Medicare fraud to 1-800-MEDICARE or the HHS Office of Inspector General at 1-800-HHS-TIPS.
