Case from 2022–2023 (U.S. Attorney's Office latest release: June 16, 2023). Published on Scammers.com Oct. 16, 2026; case status is current as of that date.
Dr. John A. Greager II, 75, of Hinsdale, Illinois, was sentenced June 13, 2023, to six months in federal prison and fined $1 million. He had submitted fraudulent claims to Medicare and Blue Cross Blue Shield of Illinois for mole removals, according to the U.S. Attorney's Office for the Northern District of Illinois [1]. He pleaded guilty in 2023 to health care fraud before U.S. District Judge Robert W. Gettleman.
How the scheme worked
According to the U.S. Attorney's Office [1]:
- Splitting procedures. Greager owned Cancer Therapy Associates S.C. in Lombard, Illinois. From 2015 to 2021, he removed several moles from a patient on a single date but billed as if they were removed on separate dates. He admitted knowing insurers paid more that way.
- Delaying lab tests. To hide the scheme, he stored the moles at his office and held off sending them for pathology testing, so the lab reports made it look as if they were removed on different dates. The release says this delayed critical testing for serious illnesses, including cancer.
- Fake charts. He created fake patient charts, including pre- and post-operative notes for procedures on dates when he was not in Illinois. After a Medicare audit, he told staff to create false records.
- The payout. Medicare and BCBS paid him about $1.7 million as a result of the scheme.
The 2022 charging document had alleged about $4.1 million in fraudulent payments. It also alleged that some services were medically unnecessary and that he removed more moles than was medically necessary [2]. At sentencing, the U.S. Attorney's Office cited the $1.7 million figure [1].

- Ask when your biopsy was sent to the lab and follow up if results are slow.
- Report suspected billing fraud to the HHS-OIG hotline at oig.hhs.gov or your insurer's fraud unit.
