Case from 2019–2021 (U.S. Attorney's Office latest release: April 27, 2021). Published on Scammers.com Oct. 16, 2026; case status is current as of that date.

John Kosloski, 57, of Beecher, Illinois, ran Diagnostic & Therapeutic Rehab, a chiropractic practice in Dolton. On April 26, 2021, he was sentenced to 20 months in federal prison for billing insurers for treatment he never provided, according to the U.S. Attorney's Office for the Northern District of Illinois [1]. U.S. District Chief Judge Rebecca R. Pallmeyer also fined him $25,000 and ordered more than $500,000 in restitution to the insurers.

How the scheme worked

According to prosecutors [1] [2]:

  1. Cash for identities. Kosloski paid Amtrak employees cash for permission to bill insurers using their personal information and their family members'. The indictment said he handed co-schemers envelopes of cash after insurers paid.
  2. Phantom patients. From 2010 to 2017, he billed private insurers, and Medicare according to the sentencing announcement, for chiropractic services to people he wasn't treating or had never seen.
  3. Volume. He submitted more than 18,000 false claims and collected more than $500,000.

"He made the choice to defraud insurers over and over again for eight years, stealing hundreds of thousands of dollars," Assistant U.S. Attorney Kelly M. Greening wrote in the government's sentencing memorandum [1].

The person sentenced

John Kosloski, 57, Beecher, Ill. [1]

  • Charges: indicted in 2019 on six counts of health care fraud; pleaded not guilty at arraignment June 3, 2019 [2].
  • Plea: guilty to one count of health care fraud (2020).
  • Sentence: 20 months in federal prison, a $25,000 fine and more than $500,000 in restitution, imposed April 26, 2021.
  • Investigation: FBI Chicago and the Amtrak Office of Inspector General.

Red flags

  • A provider offering you cash to use your or your family's insurance information.
  • Explanation of Benefits (EOB) statements for visits you never made, or for relatives who never went.
  • A clinic that bills for frequent visits while you're rarely or never there.

What to do

  1. Read every EOB from your health plan, including those for covered family members. Report unfamiliar claims to your insurer's fraud unit.
  2. Never sell or lend your insurance information. Patients who take cash can face charges too.
  3. Report health care fraud to your insurer, to the FBI at tips.fbi.gov and, for Medicare, to HHS-OIG at 1-800-HHS-TIPS. Fraud involving Amtrak benefits can be reported to the Amtrak OIG.
