Case from 2023–2025 (U.S. Attorney's Office release: May 11, 2023; later developments are from court records and other sources cited below). Published on Scammers.com Oct. 16, 2026; case status is current as of that date.
A Calumet City couple, Richard W. Pitts and Tammie J. Pitts, and a Chicago social worker, Charlo G. Jordan, were indicted in 2023. Prosecutors charged that American Warriors Inc., the not-for-profit the couple ran, received $1,164,624.61 from Medicare for psychotherapy that senior living residents never received [1]. In June 2025, the Illinois Department of Financial and Professional Regulation reported that Jordan was convicted on March 19, 2025, of one count of health care fraud [2]. Court records also show further proceedings involving the Pitts defendants in 2025, described below.
What prosecutors alleged
According to the indictment, as described by the U.S. Attorney's Office for the Northern District of Illinois [1]:
- The company. From 2019 to 2021, American Warriors Inc. said it provided psychotherapy to residents of Chicago-area senior living facilities.
- Breakfast meetings. Richard Pitts allegedly invited residents to breakfast meetings, often with guest speakers or group activities, to identify Medicare beneficiaries. Their information was then allegedly used to support false claims.
- False claims. The claims allegedly said residents had received one-hour individual psychotherapy sessions that the defendants knew had not happened.
- "Telehealth." After the COVID-19 pandemic ended the in-person breakfasts, the company allegedly collected residents' phone numbers for "telehealth" therapy and kept billing Medicare for sessions that never took place.
IDFPR's disciplinary report states that from February 2020 to April 2021, Jordan, an American Warriors employee, used Medicare information from senior facility residents to submit fraudulent claims for psychotherapy that did not occur [2].
The defendants
All three pleaded not guilty at their arraignments on May 9, 2023 [1]. The case is N.D. Ill. No. 1:23-cr-00237.
- Charlo G. Jordan, 43, Chicago, clinical social worker. IDFPR reported that Jordan was convicted March 19, 2025, of one count of health care fraud under 18 U.S.C. § 1347. It indefinitely suspended Jordan's clinical social worker license for at least one year [2]. His federal docket shows a change-of-plea hearing on Nov. 15, 2024, a sentencing hearing on March 12, 2025, and an amended judgment on March 19, 2025 [3]. We did not find the terms of Jordan's sentence in public sources.
- Richard W. Pitts, 67, and Tammie J. Pitts, 53, Calumet City, Ill. Both were charged with health care fraud [1]. CourtListener keeps separate docket records for the two Pitts defendants. One record shows a change-of-plea hearing on May 20, 2025, a forfeiture order, and a sentencing hearing on Aug. 29, 2025. The other shows a motion to dismiss filed Feb. 28, 2025, and an order on that motion on March 5, 2025. The public entries we could review do not identify which spouse each record belongs to, and they do not give plea or sentence details. We are therefore not attributing a conviction or a dismissal to either spouse. Their status is unresolved in the public sources we found as of Oct. 16, 2026. Each is presumed innocent unless and until proven guilty.
The case was investigated by the FBI and HHS-OIG [1].
Red flags (for seniors and families)
- Free breakfasts, social events or "wellness" talks where organizers collect Medicare numbers.
- Calls offering "telehealth therapy" you didn't ask for.
- Medicare Summary Notices showing therapy sessions you never had.
What to do
- Guard your Medicare number like a credit card. Don't give it out at events or over the phone to people you don't know.
- Check your Medicare statements at medicare.gov or on paper notices, and question anything unfamiliar.
- Report suspected fraud to 1-800-MEDICARE, the HHS-OIG hotline at oig.hhs.gov, or the Illinois Senior Medicare Patrol.
