Case from 2014–2020 (U.S. Attorney's Office release: July 15, 2014; later developments are from court records and other sources cited below). Published on Scammers.com Oct. 16, 2026; case status is current as of that date.

John Yousefzai and his wife, Armanouhi Arzomanian, a Skokie couple who owned Medicose Home Health Care Service Inc., were arrested on July 15, 2014, on charges that they billed Medicare for more than $800,000 in physician services never provided and paid kickbacks for patient referrals, the U.S. Attorney's Office for the Northern District of Illinois announced [1].

Outcome: All three people charged pleaded guilty, according to the court docket [2]. In September 2016, U.S. District Judge Harry D. Leinenweber sentenced Yousefzai to 24 months in prison and Arzomanian to 18 months, each with $876,888 in restitution. Referral source Wilson Narsa received a non-prison sentence. The charges against the company were dismissed on the government's motion in June 2020.

How the scheme worked

According to the indictment, as summarized by prosecutors [1]:

  1. The business: Medicose, run from the couple's Skokie home, employed four Illinois-licensed physicians to make home visits. Neither Yousefzai nor Arzomanian is a licensed medical professional.
  2. Phantom visits: From May 2008 to January 2014, Medicose billed Medicare more than $2.1 million for physician home visits and was paid more than $1.4 million. More than $1.3 million of the claims were for services not provided, causing more than $800,000 in losses.
  3. Kickbacks: From December 2010 to August 2013, Medicose paid kickbacks for Medicare patient referrals, including at least $11,000 to Narsa, who worked for a nonprofit serving elderly and disabled people.

The people sentenced

Outcomes are from the court's judgments [2]:

  • John Yousefzai, 66 at arrest, Skokie — pleaded guilty May 17, 2016, to Count One; sentenced to 24 months in prison, two years of supervised release and $876,888 restitution (judgment Sept. 8, 2016).
  • Armanouhi Arzomanian, 56 at arrest, Skokie — pleaded guilty May 17, 2016, to Count One; sentenced to 18 months, two years of supervised release and $876,888 restitution. By agreement, her surrender was deferred until her husband completed his sentence. In September 2020, the court modified her prison term to time served under the First Step Act and ordered her released.
  • Wilson Narsa, 52 at arrest, Chicago — pleaded guilty Dec. 16, 2014, to one kickback count; the October 2016 judgment imposed a two-year term of supervision and no prison time, with an $11,000 money judgment.
  • Medicose Home Health Care Service Inc. — charges dismissed on the government's motion, June 5, 2020.
